Description
Background: Hypertension affects 200M+ adults in sub-Saharan Africa, control rates below 10%.
Objective: Evaluate CHW-led task-shifting for hypertension management in Nigeria and Ghana.
Methods: Cluster-RCT, 42 centers, 3,847 patients, 18 months. CHW-led BP monitoring, adherence counseling, lifestyle coaching.
Results: SBP reduction 12.4 vs 4.2 mmHg (p<0.001). Control rates 42.1% vs 18.7% (RR=2.25). Adherence improved 34 percentage points.
Conclusion: CHW-led hypertension management is effective and scalable for sub-Saharan Africa.
